원구한의대 한방병원에 내원하시는 환자분들은 대부분 요통 등의 근골격계 질환과 더불어 신경정신과 질환도 호소하는데, 이 중 흔한 경우가 불면증이다. 과거 LA의 양방 정신과 클리닉 내의 침구과에서 근무하던 때에도 이차성 불면증을 호소하는 환자들이 많았고, 이 클리닉에서 자주 사용하던 양약의 흔한 부작용으로 나와있다.
불면증의 한의학에서 내리는 진단은 환자의 상태에 따라 다르게 나온다. 잠에 들지 못하는 양상들, 예를 들어 잠에 들기 어렵거나, 잠에 쉽게 들지만 쉽게 깨거나, 잠을 유지하지만 꿈 등으로 방해를 받는 등의 양상에 따라 진단이 달라질 수 있다.
이에 더불어 불면증과 수반되는 다른 증상들에 따라서도 진단은 세분화 될 수 있다. 심정적으로 화가 많거나, 쉽게 신경질적인 성품, 혹은 생각이 많고, 쉽게 무서워하는 성품 등의 본성적인 부분도 같이 참고 되어야 한다. 또 내과적인 질환들, 심혈관계의 고질병의 유무, 소화기계통의 불편한 점들, 호흡기 및 피부 질환에 대해서도 정보를 취합하여 종합적인 한방 진단을 내리며, 이의 불면증의 양상과 비교하여 세부 진단을 내린다.
이에 따라 혈자리의 조합으로 침치료가 구성되고, 약재들의 조합으로 탕약이 구성된다. 예를 들어, 쉽게 신경질적인 성품의 불면증 환자가 내원하는 경우, 설진과 맥진, 촉진으로 한의사 본인이 내린 진단을 확정하고, 이에 관련된 장부의 혈자리, 예를 들어 족궐음 간경의 원혈인 태충 같은 혈자리와 이에 상통하는 수양명대장경 원혈인 합곡과, 표리관계로 부합하는 족소양 담경의 낙혈인 광명 등을 조합하여 침치료를 구성한다. 환자에게 한약을 처방하는 경우, 위의 진단이 내려진 환자에게 간의 경락으로 가는 약재들, 울체 되어 있는 기운을 풀고, 몸을 가볍게 하며 음의 기운을 돋구는 시호나 백작약 등이 포함된 약재들로 구성된 탕제를 선택하게 된다.
한의원에서 인종과 남녀 노소를 불문하고 흔하게 만나게 되는 불면증 환자들에게 위와 같은 방법으로 불면증을 진료해왔고, 환자들의 수면의 질이 높아졌고, 이에 수반된 다른 질환들과 양약 복용 필요성이 감소됨을 볼 수 있었다. 주치의의 처방에 더불어 자연친화적이고 자가치유의 능력을 활용하는 한방 진료도 고려해 볼 가치가 있다.
While the chief complaint of most patients in the Wongu University of Oriental Medicine Clinic is musculoskeletal issues, such as low back pain, they also often address their psychiatric conditions such as insomnia. While I worked at the Acupuncture Department in the Psychiatric Clinic in West Hollywood, many patients complained about their secondary insomnia which is also a common side effect of some of the psychiatric medications.
Diagnosis in the Oriental Medicine of insomnia is different up to each patients’ condition. For example, quality of sleep, such as difficulty to fall asleep, easy to fall asleep while often awake, and easy to stay asleep but disturbed by dreams would be some of the concerns of the different types of insomnia.
In addition, other complications with insomnia can also influence the Oriental Medicine diagnosis. Oriental Medicine Doctors (OMD) need to get to know patients’ personalities including whether the patient tends to have more anger, easily upset, worried about many things, or easily to get afraid. To complete the diagnosis, OMD also includes information regarding internal medicine conditions, cardiovascular chronic concerns, gastrointestinal concerns, respiratory and dermatological concerns. This conclusion would be compared with the pattern of insomnia to finalize the diagnosis.
Based on the diagnosis, OMD creates a combination of acupuncture points to perform acupuncture treatment and a combination of single organic herbs to provide herbal formula. For example, when the insomnia patient with a personality of easy to get upset personality visit the clinic, OMD performs tongue, pulse, palpation diagnosis, and proceed treatments with the primary point of Foot Jue Yin Liver meridian which is LV3, the primary point of Hand Yang Ming Large Intestine Meridian which is mutually connected to Liver Meridian, LI4, and adding a Connecting point of Foot Shao Yang Gall Bladder Meridian which is Exterior & Interior relationship with Liver Meridian, GB37. For Herbal formula, based on the diagnosis above, OMD would consider single herbs that goes to Liver Meridian, especially with the function of relieving stagnated condition, make the patients feel lighter, inducing Yin energy, such as Chai Hu, Bai Shao Yao would be relevant.
Regardless of age, ethnicity, or gender, it’s very common to encounter insomnia patients in the acupuncture clinic. We have provided treatment as above, and we have found that patients are able to improve their quality of sleep and reduce the necessity of medication.
